Payroll Tax Forms
If you do not already have an Employer Identification Number (EIN), you will need to apply for a number by filling out a
SS-4
IRS Form 941
Here is the link to the payroll tax form 941. It is in Adobe Reader, so you can type all your information and then print it.

IRS Form W-4
If you have employees, you need to have a completed W-4 form for each employee on file.

IRS W-9 Form
For every independent contractor that you paid more than $600 in a year, you should have a completed W-9 on file.

You can NOT download a usable 1099-MISC to send to those independent contractors, but here is a sample of this form and the number to call to get your 1099s and 1098s.

You can NOT download a usable W-2 and W-3 forms to use for your employees, but here is a sample of this form and the number to call to get your W-2s and W-3s.
